Multimodal AI platform recognized for its potential to reduce assessment costs,accelerate completion, and expand access for Medicare and Medicaid populations 

PITTSBURGH, PA, September 11, 2026 — PRLog — Ten Ten Ten, a nonprofit organization dedicated to improving U.S. health outcomes while reducing healthcare costs, today announced that it has awarded Provisional Certification to Cara AI, a multimodal platform designed to help health plans and care teams conduct home-based health and functional assessments.

The certification covers Cara’s Long-Term Services and Supports functional assessments as the primary scope and its Health Risk Assessment module as a secondary scope. Cara allows patients and caregivers to complete assessments through voice, text message, web-based interactions, and guided photo or video review. The platform identifies potential mobility, medication, home-safety, and daily-living risks and converts the information into structured documentation for care teams.

Ten Ten Ten Certification evaluates healthcare products, services, programs, and policies against three pillars: Health Outcomes, Economic Benefits, and Equity. Provisional certification is intended for pilot, early-market, or emerging solutions that present transparent preliminary evidence and commit to developing confirmatory real-world data.

“Provisional certification does not mean every projected outcome has already been proven,” said Bhargav Raman, MD, MBA, cofounder and Certification Lead at Ten Ten Ten. “It means the solution has presented credible evidence, has been transparent about what is measured and what remains to be validated, and has a responsible plan for generating the evidence needed for full certification. Cara met that standard.”

Ten Ten Ten’s review found that Cara satisfied the certification standards for Economic Benefits and Equity and met the provisional pathway for Health Outcomes. Operational evidence indicated that the platform can maintain assessment quality while lowering costs, accelerating turnaround, increasing completion, and substantially expanding coordinator capacity. The review also highlighted Cara’s multilingual, SMS-first, app-free design and its ability to reach dual-eligible, rural, limited-English, and low-broadband populations.

“Too many patients experience delays or miss assessments entirely because traditional in-home models are constrained by geography, language, staffing, and cost,” said Renee Dua, MD, founder and CEO of Cara AI. “Cara was built to make assessments more accessible and consistent while giving care teams timely, structured information they can use to identify needs and coordinate care.”

Cara’s platform reaches members through voice calls, text messages, email, and web-based interactions, automatically structures and codes responses, and alerts care teams when identified needs require action.

The provisional designation reflects an important distinction between Cara’s measured operational performance and its longer-term outcomes projections. Direct reductions in emergency department utilization, hospital admissions, skilled-nursing use, and total cost of care have not yet been independently demonstrated. Cara has committed to a confirmatory study designed to measure those outcomes using an intervention group and a concurrent, risk-matched control group.

“Healthcare AI should not be certified because it is novel or because its projections are attractive,” Raman added. “It should be judged on whether it can improve or maintain quality, reduce total system cost, expand equitable access, disclose its incentives, and produce evidence that can withstand scrutiny.”

Cara will be required to complete annual re-attestation of its progress across health outcomes, affordability, and equity, and to notify Ten Ten Ten of material changes to its pricing, ownership, data use, or business model. Provisional certification may be reconsidered if confirmatory evidence is not produced within the required period or if subsequent findings do not support the certified claims.
