
PITTSBURGH, PA, October 2, 2026 — Ten Ten Ten, a nonprofit organization dedicated to improving U.S. health outcomes and reducing healthcare costs, today announced the release of Maximizing Clinical Operations: The Operating System of a Value-Based Primary Care Practice, a new implementation guide for independent primary care practices.
The 89-page guide helps practices turn value-based care goals into reliable daily work. It covers patient empanelment and payer attribution, working at the top of each team member’s scope, pre-visit planning, morning huddles, follow-up on results and referrals, transitions of care, and patient outreach. Workbook exercises help teams adapt each workflow to their own practice.
“Value-based care succeeds or fails in the work between visits,” said Jenn Block, PhD, Ten Ten Ten’s Chief Architect, Value-Based Care. “A patient needs a follow-up call after discharge. A referral needs to reach the specialist and come back with a plan. This guide gives practices a way to assign that work, track it, and make sure it gets done.”
The guide begins with a foundational task: reconciling the patients a payer attributes to a practice with the patients the practice actually sees. It then shows how to prepare for visits, use a brief daily huddle to coordinate the team, and create clear ownership for test results, orders, referrals, and patient messages. A transitions-of-care workflow sets out the steps following a hospital discharge.
To help practices sustain these routines, Maximizing Clinical Operations provides a daily, weekly, and monthly operating schedule and a small dashboard of measures such as pre-visit preparation, referral completion, and post-discharge outreach. Rather than asking practices to change everything at once, it includes a reusable Plan-Do-Study-Act worksheet for testing one workflow on a small scale before expanding it.
“Knowing which patients you are responsible for is the starting point,” said Bhargav Raman, MD, Ten Ten Ten cofounder and Lead, Clinical Strategy & Analytics. “From there, the practice needs systems that reveal what has been missed and make the next action clear. The goal is dependable care that does not rely on someone remembering every task.”
